If you have a prayer request or thanksgiving that you would like prayed, please complete the following form. These requests are held in the strictest of confidence and are not shared with others; this form is emailed directly to Pastor Kozak. However, if you would like the name(s) added to our public worship life, please indicate this. Also, unless otherwise indicated, the name(s) are removed after four weeks. |
